Preoperative evaluation of melanoma thickness by dermatoscopy
نویسنده
چکیده
Preoperative evaluation of melanoma thickness using dermatoscopy has been published in only few studies in the world, and it is done only experimentally. In daily praxis, none of the published methods is applicable. Great importance for preoperative determination of tumor thickness lies in the fact that surgical margins and sentinel node biopsies are determined only by melanoma thickness (reported as Breslow index). As the skin is semitransparent medium, the two borders, i.e. depth of the skin, were investigated in this study: 0.75 mm and 1.0 mm. Combining clinical (tumor elevation, longer diameter and surface area) and dermatoscopical characteristics (network, hypopigmentations, streaks) of 40 melanoma, we constructed an algorithm for prediction of melanoma thickness. Concerning melanoma thickness less than 0.75 mm, in 83.3% cases correct pre-operative assessment of melanoma thickness was done using the given clinical and dermoscopic criteria. In melanoma equal or thicker than 1.0 mm in 87.5% of cases it is possible to make a correct pre-operative assessment of melanoma thickness using the given clinical and dermoscopic criteria. The model of clinical-dermoscopic melanoma thickness assessment is extremely applicable and reproducible model in practice; does not require expensive devices, additional examinations or long experience in dermoscopy, thus being applicable in various centers where melanoma is treated. Dermatoscopic prediction of melanoma thickness using latent trait analysis and likelihood ratios.
Breslow thickness and Clark level can be used to determine surgical procedures for cutaneous malignant melanoma and patient eligibility for experimental adjuvant therapy. Efforts to predict the thickness of melanomas using dermatoscopy have focused on differences between single dermatoscopic findings. The aim of this study was to develop a method for preoperative identification of melanomas of ...
متن کاملClinical and dermatoscopic diagnosis of malignant melanoma. Assessed by expert and non-expert groups.
We investigated the nosographic and diagnostic probabilities and likelihood ratios of dermatoscopy in order to evaluate the method's role in decision-making regarding melanoma. Clinical slides and dermatoscopic photos were obtained from 232 patients referred for dermatoscopy. Four dermatoscopy "experts" and 5 "non-experts" assessed the slides. Diagnoses were compared with histopathology. Sensit...
متن کاملThe dermatoscopic ABCD rule does not improve diagnostic accuracy of malignant melanoma.
The dermatoscopic ABCD rule has been suggested to improve diagnostic performance regarding cutaneous malignant melanoma. Using this rule, a total dermatoscopy score is calculated from the presence of various dermatoscopic elements. A total dermatoscopy score above 4.75 signifies possible and 5.45 probable melanoma. We compared the diagnostic accuracy of dermatoscopy with and without the use of ...
متن کاملBalloon cell melanoma: a case report with polarized and non-polarized dermatoscopy and dermatopathology
Balloon cell melanoma is a rare melanoma subtype, with only one previous case with dermatoscopy published. It is often non-pigmented, leading to diagnostic difficulty, and there is a tendency for lesions to be thick at diagnosis. We report a case of balloon cell melanoma on the forearm of a 61-year-old man with both polarized and non-polarized dermatoscopy and dermatopathology. It presented as ...
